Rambler's Youth Sports Registration Forms

I understand that all children are to be picked up after practice and or their games when completed. Practice is over at 8:00 P.M. All children should be picked up between 8:00pm and 8:15pm (NO LATER) from the field during summer camp and the regular season. The Sharon Hill Rambler Organization will not be held responsible for children left unattended after 8:15p.m.

Medical Release: I, in my own behalf and on behalf of my child named on this registration form (hereafter known as Minor), acknowledge and agree that participation in Football/Cheerleading may subject Minor to the possibility of physical illness or injury (minimal, serious, catastrophic and/or death) and that I, in my own behalf and on behalf of Minor, acknowledge that Minor is assuming the risk of such illness or injury by participating in this sport. In the event of such illness or injury, I authorize the Sharon Hill Ramblers to obtain necessary medical treatment for Minor and hereby, in my own behalf and on behalf of Minor, release and hold harmless the Sharon Hill Ramblers in the excercises of this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of Minor for any illness or injury that Minor may sustain during their participation in this sport and while traveling to and from the site for a game whether or not the game actually occurs.
MEDICAL/EMERGENCY CONTACT INFORMATION
There will be no cursing/swaering and coach's or other players.

CODE OF CONDUCT

The Ramblers' Football and Cheerleading Association is a non-profit organization whose purpose is to provide to the community youth the opportunity to participate in supervised sports; to organize athletic teams; to teach basic principles of the various sports; to arrange, supervise and conduct sporting activities and contests; and to instill sportsmanship and civic responsibilities in the youth.

The Sharon Hill Recreation Board Code of Conduct applies to all players, coaches, and parents

The Ramblers' Football & Cheerleading Association will not tolerate any arguing, negative, offensive or threatening comments and/or gestures directed towards officials, referees, players, coaches or spectators of either team.

There will be no use of tobacco and or alcohol products around the team bench during practices or games.

All borough ordinances will be enforced at all Ramblers' Football & Cheerleading Association activities. Members of the organization or organization sanctioned groups who violate these ordinances will be referred to the Executive Board for disciplinary action

Any member of the Ramblers' Football & Cheerleading Association who deliberately and willfully destroys club equipment or property will be held responsible for restitution and will be referred to the Executive Board for disciplinary action

Coaches are required to obtain an Act 34 Clearance (background check).

Any coach, player or parent who violates any rules or by-laws of the Association will be subject to disciplinary action
by the Executive Board of the Ramblers' Football and Cheerleading Association. This action could consist of:

Ø First offense: Player or parent handled at the discretion of the coach OR if a coach is involved - handled at the
discretion of the Executive Board

Ø Second offense: Suspended from the Association pending review of the Executive Board with the possibility of
expulsion

Ø Third offense: Expulsion from Association for one (1) year from time of expulsion (reinstatement pending
Executive Board review and approval)

By checking this box I/We acknowledge that the Ramblers Football & Cheerleading Association's acceptance of my child to play in this organization is conditional and subject to our acceptance of this Code of Conduct and agreeing for ourselves and on behalf of other members of our family, to be bound by its provisions and to be subject to any disciplinary actions that might result.
