COACHES CERTIFICATION AND AGREEMENT

I certify that the facts set forth in this application are true and complete to the best of my knowledge. I authorize Central California Krush to make an investigation of any fact set forth in this application. I also understand that this organization will require a background check from the ASA (Amateur Softball Association)and possibly a state back ground check before I can coach/asst coach a team. I also understand that I will be required to attend Krush Board meetings on the dates scheduled by Krush board.
