2019 - Fairmount Player Registration Form

Fairmount Park 'A' League
WAIVER INFORMATION
The Player whose electronic signature appears below agrees that he/she will, for the calendar year 2019, be a member of the team listed below, unless said Player is released by the Sponsor in the manner dictated by the Fairmount Park 'A' League Bylaws.

This executed agreement is to be forwarded to the Commissioner of the Fairmount Park 'A' League. A copy can be requested or retained by the Sponsor and/or the Player.

The Sponsor may terminate this agreement at any time by using the league authorized Release form. This form must be filled out in triplicate. One copy is to be retained by the Sponsor, one copy given to the Player, and the final copy forwarded to the Commissioner of the Fairmount Park 'A' League. The player may also request termination of this agreement at any time. In order to obtain Release from this agreement, the Player must return any and all uniform pieces given to him by the Sponsor (unless purchased by the Player) and he must also pay all moneys (team dues) owed to the Sponsor. If the calendar year ends without the satisfaction of this agreement (return of uniform and payment of moneys owed), the player will remain bound to the Sponsor’s team until such responsibilities are completed. It is the responsibility of the Player to request receipts from the team Sponsor when paying dues, as well as when accepting and returning team equipment and uniforms. These receipts can be used as proof and verification in case of a conflict during a request for release. The Player agrees that his total financial obligation for this contract (listed below) and that the following uniform pieces have been given to him (team listed below).

In the event the undersigned Player is guilty of violating this agreement, by becoming affiliated with another Fairmount Park 'A' League baseball team without having been properly released from this agreement, the Fairmount Park 'A' League club will be penalized under the forfeit rules of Article 20, Paragraph C, of the Fairmount Park 'A' League Bylaws.

By signing this agreement, the Player and Sponsor acknowledge that the Fairmount Park 'A' League is a play-at-your-own-risk league. The undersigned Player accepts full responsibility for any injuries he obtains or damage to his own personal property that occurs before, during, or after games, practices, or other league functions.

The Player and Sponsor also acknowledge that they player is at least 18 years of age at the time of signing this contract, as only 18 year old or older players may play in the Fairmount Park 'A' League. Any failures to meet the 18 years of age requirement are the sole responsibility of the player and the sponsor and not the Fairmount Park 'A' League or any of its other officers, Board Members, players, managers, sponsors, etc…
