I, the prospective Morris Majors player named above request that he/she to be placed onto a Morris Majors Baseball team for the upcoming current regular season.

I understand and agree that the Morris Majors Board of Directors may or may not approve this request.

I understand and agree that the league President of the Morris Majors is the final authority in determining whether or not I will be approved for this waiver.

I understand and agree that if the league President does not approve this request, it cannot be appealed except to the league President.

I understand and agree that if this request is approved, then I will:
• Follow the Bylaws of the league and will face all consequences if I do not.
• Commit fully to that team.
• Be loyal to that team and will not seek another team until speaking with current team's coach.
