2019 New Team Application Use this form to submit a NEW TEAM for the upcoming season. Mason Amateur Baseball (MAB) will use this information to update its email distribution and phone list. It also helps us plan for the upcoming season. We will use this information as part of the process to join MAB. Submission of this form does not guarantee membership in MAB. |

NYSCA Membership Form Use this Form to sumbit your NYSCA Manager’s Certification Member Number. MANDATORY FOR ALL HEAD COACHES AND MAIN ASSISTANT: Managers and their main assistant coach must register their NYSCA card number with the Mason Amateur Baseball before taking the field in 2018. Failure to do so will result in games and practices not scheduled. |
