2018 Little League Player Registration Form This form allows one player to register and provide contact and parent/guardian information. If you have more than one player to register, please use "Add Another" feature. If desired, you may submit these forms (Birth Certificate plus 3 proofs of residency) via email by scanning them and emailing them to National_LittleLeague@yahoo.com Please put the player’s name in the Subject line of the email. If you choose not to scan and email the birth certificates and residency forms, you must bring copies during the registration times listed on the Welcome Page. ALL registrants must make payment during the registration time period. We hope to have a quick payment line will available.
