Must submit a copy of your certificate of insurance naming NPMYAC LL, PO Box 629, Patchogue NY, 11772 as additional insured to pmtraveltournaments@gmail.com. Each team must have a copy of the team roster with names and birthdates and copies of birth certificates available at the tournament. |
