In order for your application to be processed completely and accurately from this website, it is recommended that you take the following actions: 1. Enter your Email Address, in the box below. 2. This Application is in 2 parts Team Application and Team Roster. It is mandatory to complete both forms. We will confirm receipt of your application via the email address you provide: Your information will be saved and sent to the tournament director. The information provided is private and will not be shared or sold to any other organization. After you have completed both applications, click on the SUBMIT button. Please review for accuracy before clicking on the submit button. Once submitted the system will take you to your completed application for printing. Print the page that follows (it will appear as if it has been filled out - with your answers to the application questions in place). Follow the directions that appear regarding your payment. Send the printout along with your payment to the address shown.
