ROY HOBBS NWOBL UMPIRE EVALUATION FORM This form is to evaluate the umpires for the 2014 Roy Hobbs NWOBL season. Please fill out this form each week for both of the umpires that officiated your game. Please get the name of both umpires and fill out this form when you get home. After filling out the forms, one for each umpire, please hit the “Submit” button at the bottom of the form. The forms will be automatically sent to Ollie LaCourse, lead umpire, and Troy McDaniel, President of the Roy Hobbs NW Ohio Baseball league. All forms will be kept on file. Thank you |
