2017-2018 Drivers Release Form
All PARENT drivers must have this form completed and on file before driving any other dancer to or from an off campus event.- 2017-2018 Driver permission for non parent to drive dancer
All parents must give written permission for their driver to be driven by someone other than themselves to and from an off campus event. This form must be turned in when that need should arise.
