Here is the preferred form for use for your child's mandatory physical exam.
Please take it with you to your physical so we can have a copy for our records.

Just like our Coaches, our parents should abide by a code of conduct to allow the process to run smoothly. We know that you may not always agree with a referee's call or a coach's decision, but this issues must be handled appropriately.
