Each team have 8 sheets for each regular season games played. This sheet keeps record of players game attendance.
GMYFL Official Player Registration & Code of Ethics Form
Form must be named in underlined spaces by each GMYFL organization, completed and signed by all GMYFL parent/guardian and players and be placed in your team book. Form may be altered to reflect your clubs specific year, cost, fees and special instructions but the body and nature of the disclaimer shall not be altered.- ID Certification Form for Coaches
Coaches - Copy and Paste the players name and DOB from your official roster to this form, and bring both to your team certification.
