Parent Etiquette Contract (Required)
This contract outlines expectations and rules for the parents of student-athletes. Parents should read, sign, and return this contract if allowing their student-athlete to participate.

"Try Out" Waver — Student
Must be completed for a student to participate in one practice to "try out" the team without registering or paying fees.

"Try Out" Waver — Adult
Must be completed for an adult to participate or volunteer with the team without registering or paying fees with the Utah High School League.
