Main Registration Form
This is our form. This is needed along with at least one of the 4 registration forms below

OFPD Football Registration Form
This form is needed along with the Main Registration Form if you have 1 football player

OFPD Cheer Registration Form
This form is needed along with the Main Registration Form if you have 1 cheerleader

OFPD Additional Football player form
This form is needed along with the Main Registration Form and OFPD Football or Cheer Registration Form if you have more than 1 participant

OFPD Additional Cheerleader form
This form is needed along with the Main Registration Form and OFPD Football or Cheer Registration Form if you have more than 1 participant

Background Check
It is Oak Forest Park District policy that any coach or assistant coach fill out a Background Check Consent/Release form. Regardless of other organizations in which you may volunteer, please fill out, sign, date, and return the form to the Park District Office as soon as possible.
