This form is needed to have your sports physical performed by your own doctor. Print this form and take it to your doctor to fill out. You will need a medical exam and this form before you can participate. All exams must be performed after March 26, 2021 AND within 4 months prior to first day of practice. NO EXCEPTIONS! Dr stamp REQUIRED on form.
