Attached is the COVID release form that each student athlete will need to fill out and bring into the athletic office with their physical. This form is NOT part of registration.

TRANSPORTATION & REGISTRATION FORMS
These forms must be completed plus the $90.00 fee paid before your swimmer is eligible.

ALTERNATE TRANSPORTATION FORM
This form MUST be PRE-APPROVED by the coaches turned into coaches the day of the meet if your athlete can't ride the school bus to a swim meet and back to RUHS with the team.
