To be used to purchase team insurance from American Youth AYSSA. Costs are $240 for your organization.

COACHES - PA CHILD ABUSE (CY113)
To secure PA clearance of child abuse. Although not done annually, it cannot be more than 5 years old (2019 or newer).

COACHES - PA CRIMINAL CHECK (SP4-164)
To be secured by all persons with a team who have an involvement with players. A copy of this form must be presented at the team's registration. While the form does not have to be done annually, it cannot be older than five years (2019 or newer). The original should be held by the initiator or head coach.
