2016 Volunteer Application
All persons volunteering with SPGLL must pass a background check with the United States Department of Justice National Sexual Offender Registry along with the national crime records through Little League Baseball, Inc. website. All persons having any contact with children within our league must fill out a Volunteer Form annually. If you would like to assist on any level with the children in our program you must fill out a Volunteer Form annually and submit it along with a copy of a valid driver license or government issued photo identification.

School Enrollment Form
If parents do not reside within our boundary, but the player goes to school within the boundary, this form will need to be filled out by a school administrator in order to participate.
