AYF Participant Packet
To be rostered, each player must submit a completed AYF packet.
In addition to this packet each player will need the following:
Physical dated in 2024. The doctor must complete the medical clearance form in the AYF packet (page 5).
**This is the only form accepted for physicals**
Copy of the players birth certificate
State issued ID for your player
**We will not accept passports this year. If you have military IDs, we can accept them. However, it is strongly suggested for you to plan to get a state ID since we must retain the ID through the entirety of the season.**
