CDC Considerations for Youth Sports
As per the CYFL Back to Sport Health and Safety Plan, all coaches, staff, parents/guardians and participants must review and comply with the CDC Considerations for Youth Sports for the 2020 Season.

COVID-19 Screen Form
This Screening Form must be thoroughly completed by a parent/guardian for each athlete before any practice, event or game. Failure to complete and turn in a form for each practice, event or game, will result in denied participation. Please cooperate!

CYFL Return to Sports COVID-19 Health & Safety Plan
All Coaches, Staff, Parents, Guardians and Participants should review and become familiar with this Plan, as strict compliance with this plan will be required for participation in the 2020 Season.
