WPASL Referee Report
This form should be printed by the HOME team and given to the referee before each game. The referee will fill it out, and return to the HOME team after each game. The HOME team will then send this form to the league president within 12 hours of the completion of the game.

WPASL Managers Report
This form should be printed and filled out by BOTH teams before and during the game. The official game roster will be put on this sheet and referees will sign in players before the game. BOTH managers are responsible for keeping track of statistics, and BOTH managers must send a final completed form to the league manager 12 hours following the completion of the game.
