The La Porte Park and Recreation Department now requires a birth certificate to be on file for all players within their youth sports leagues (t-ball, boys baseball, and girls softball).  If you have not already provided your child's certificate, please take it to the Park Office, 250 Pine Lake Avenue, La Porte, IN.  A copy of the birth certificate may also be emailed to the Recreation Director at pcarroll@cityoflaportein.gov. 

NOTE:  Your child will not be placed on a team until a birth certificate is on file.
