2020 Physical/Medical Form This form must be dated after January 1, 2019 and then submitted to CFA.
No other forms are acceptable. Section II must be completed in its entirety ONLY by a Licensed State Examiner (medical doctor, nurse practitioner, etc.)
Mid South Regional Site The Mid-South Region of Pop Warner Little Scholars operating in various places including Tennessee, Alabama, North Carolina, South Carolina, West Virginia, Virginia and Kentucky. Aside from this fact, their mission is to provide an environment that is safe where people can be part of a team sports or other related activities. In addition, while letting a person enjoy the benefits of participating in sports, they also made sure that young minds are being taught practical wisdom and skills that can be utilized even after finishing the program. I’m so excited to try the training especially during this time where children specifically of my age are heavily preoccupied with gadgets and the internet.
