  • PA Criminal Background Check
    This form can be paid for and completed online. Results may be immediate. If so, print the background check, copy it and complete the Child Abuse form and mail together. The cost of this clearance is $10.00. If results are not immediate, write down the control number and your name exactly as you typed it in the request. You will need to check daily for results. Once available, copy and send with Child Abuse form.

  • PA Child Abuse Criminal Record Check
    This form must be printed, completed, and mailed with the results of the Criminal Background check. The fee for this is $10.00, but is free if this is your first time running this check.
