All coaches or adults that are involved with the Lady Knights program must have their Act 33/34 Clearances processed prior to taking part in any team function on the field. The following steps must be completed:

1. Obtain a PA State Criminal Record Check for $10 online and PRINT out the "Certification Form".

2. Give a copy of these results to your organization's manager and continue to step 3.

3. If your background check is NOT urgent, complete this form for the Child Abuse History Clearance and send it in with a copy of the results in step #1 along with a $10 money order made payable to Department of Public Welfare to the address listed on the form.

4. If your background check is URGENT, then complete step #3 above by sending it OVERNIGHT to the following address. Be sure to include a return OVERNIGHT envelope depending on how quickly you need it.

5. Once you obtain the results from step #3, give a copy of these results to your organization's manager.
