2023/24 Track Season Forms

You must register by following the below process to participate with PVHS Track and Field for both summer 2023 AAU, and PVHS Track and Field season beginning in January 2024.

Please follow the following steps for registration -

2 - NFHS Courses - https://nfhslearn.com/courses - On the left margin, select "student", "all" and "free".  From there, student athletes need to complete the following three (3) courses:

- Concussion in Sports

- Heat Illness Prevention

- Sudden Cardiac Arrest

When completing your athleticclearance.com registration below, parents and student athletes will need to acknowledge by electronic signature that the 3 courses noted above were completed.

All athletes and their parents/guardians must fill out the following at https://www.athleticclearance.com in order to participate in winter AAU practices with the team.  Athletes will not be allowed to start official practice in January 2024 or compete until these forms have been completed and verified (NO EXCEPTIONS).  

If you have a valid EL2 doctors physical form from last season/year (not expired and less than 1 year from date of exam) you are cleared to participate in summer practices until said form has expired. 

New PVHS Athletics policy and best-practice is All athletes must be “cleared” in order to participate in winter practices.  If you plan to train with us this winter, get the registration and clearance done ASAP!  The current forms are uploaded to the PVHS Athletics’ page at https://www-pvhs.stjohns.k12.fl.us/athletics/

 

* To complete this clearance in full, you should have a fully signed and scanned EL2 physical form from your doctor and reviewed the courses in bullet #2.  All of this will be completed through the https://www.athleticclearance.com or follow the links from our PVHS Athletics page.  A blank copy of the EL2 form is also available on our Athletics page or by clicking https://www-pvhs.stjohns.k12.fl.us/athletics/wp-content/uploads/sites/2/2014/11/PHYSICAL-EL2-FORM.pdf

Team Fees

It is important to note that the St. Johns County School District does NOT allocate funds to any county high school athletic program.  The team will collect a fee at the beginning of the season to cover the estimated cost of running the team.  These fees cover the following: team shirts, entry fees, banquet, travel, and other miscellaneous expenses that occur throughout the season. 

Pay to Play fees for 2024 season - You can also join the boosters by adding an additional $75.00 fee to the below cost and join the boosters.  The boosters help with cost of your students sport insurance, helps provide scholarships and other cost in regard to Athletics at Ponte Vedra High School.

$250.00 per member

Payment is due by February 16, 2024.  

They have also added an online payment option to help cover the convenience fee for using Paypal, if not selected, these cost come from the track & field fund.  

IMPORTANT: Please include the name of the athlete(s) for reference either written on the check or in the special instructions field on PayPal.  Please click on the below link to pay your fees through the Booster Club.  Scroll down to the fees associated with Track and Field.  You have the option to join Boosters (which we highly encourage) or pay the direct amount with no booster membership.

Optional Forms

The following forms are not mandatory for all but may be required, as necessary:

Ponte Vedra Sharks Booster Club

Membership is entirely optional, but we would like to encourage everyone to join the Ponte Vedra Sharks Booster Club. The St. Johns County School District does not allocate funds to any county high school athletic programs and without them it would not be possible for us to compete. Of note, for every Track & Field team family that joins the Ponte Vedra Sharks Booster Club and designates their membership to the Track & Field team, the Track & Field team directly benefits with 85% of each membership going directly to the Track & Field team. The remaining 15% goes into a general fund. If at least 75% of Track & Field team families join the Ponte Vedra Sharks Booster Club, the Track & Field team gets 90% of each membership instead of 85%. This is why your membership is so important. Please fill out the 2018 -2019 Booster Membership form at -
