Stride Physio and Performance is excited to be partnering with SCMF again this year for concussion
baseline testing!  Baseline testing is mandatory for Bantam aged players and are very strongly
recommended for players in all other age groups. Baseline testing is completed yearly to ensure
accurate information is on file for your growing athlete.

The Physical Therapists at STRIDE are certified providers of the 
Complete Concussion Management Inc Program.
This means they are specially trained and certified to provide:
 

- comprehensive baseline testing to athletes, individually or as teams
- coach/trainer/physician/parent concussion education sessions
- comprehensive concussion treatment that includes return to learn, sport, and work protocols
- complete concussion treatment for individuals with post-concussion syndrome

 

Group Testing Rates:
Cost is $60 for age under 12 and $70 for over age over

 

Important points:
• This is a yearly test as young athletes change a great deal during that time. So, even if your athlete
was tested at the start of last season, it is time to update this information.

• Our group testing days are available at a discounted rate; these days are streamlined into stations for
effective flow of testing. Our therapists will NOT have time during these days to discuss the process
with parents. If you have questions about testing, please feel free to chat with with our office
administrator or visit https://completeconcussions.com/services-courses/baseline-testing/. If you wish to
have additional time to discuss the process with a therapist directly, you must book a regular baseline
testing treatment outside of group testing times.

• The concussion baseline test is a physical therapy service and covered as such under private health
insurance. There may be a portion of the fee that is not covered on regular testing days. Check your
individual policy to see if a physician referral is required for reimbursement.

• Those families that fall within Saskatchewan’s low-income cutoff may apply to the assistance program
(form attached.) This MUST be completed (with all required documents) and sent to
pamelagccm@gmail.com a minimum of 2 weeks prior to your testing day.

Booking tips:
• Book online at https://hityourstride.janeapp.com/#/physical-therapy or call 306.778.7770.
• Do not book a baseline test if the athlete has been diagnosed with a concussion within the
last 3 months or if you suspect they have a current concussion.
You MUST book a group testing treatment in order to be eligible for the discounted rates. When
booking online, double check that you have selected the group testing, as well as, the appropriate age
group. Group testing rates will not be honored due to errors in booking.
• Group testing times (discounted rate) are limited; book early!
• Be sure to book the appointment in the name of the athlete to be tested. You will need a separate
account for each child.


The day of testing:
• Attend the testing day both physically and mentally rested.
• If you have not been tested with us before, please come at least 15 minutes early to register in the
Complete Concussion Management system.
• Athletes will be tested one-on-one with our therapists to allow for the most accurate results.
Parents are welcome to remain in the waiting room or drop the athlete off. Rest assured, all STRIDE staff
are Safe Places youth certified.
