I do NOT need copies of your insurance or birth certificates.

You DO need to have copies of the above with you at all times while playing in the tournament.

The tournament DOES NOT need to be listed as additional insured on your insurance. You just need to have valid insurance for your team. Have a copy with you at all times.

Rosters - FName, LName, Uniform #, Date of Birth

Rosters can be in any medium...Word, Excel, Email, Text, Pic, Etc.
