1. Original (not a copy) of the 2016 Pop Warner Physical Fitness & Medical History Form.  Physical must have been completed in 2016.    (Must be on Pop Warner 2016 Physical Form)
2. Original 2016 Pop Warner Participant Contract and Parental Consent Form
3. Copy of Birth Certificate
4. 3 Copies of 2015-2016 Final Report Card (Must be full year report card)
5. Wallet size picture
