REGISTRATION FEE FOR TINY MITE LEVEL & UP: 
 
$100 for first child, $90 for second child 
$80 for each additional child
 
+ $75 Workbond per family
(To be returned upon completion of work bond hours & uniform/equipment return) 
All levels Flag - Varsity
+ $25 Canning bond
(Will be returned upon participation in canning fundraiser) 
Tiny Mite –Varsity level 
 
*Please bring separate checks for registration, work bond, and canning.*

Flag Cheer – Age 5
Flag Football – Ages 5 & 6
 
 
REQUIRED PAPERWORK:
Due prior to start of season
-Birth Certificate 
-Pop Warner Medical Release form completed and stamped by child’s physician 
(must be dated 2017 and before August 1st) 
-Recent wallet size photo 
-2 Copies of Final report card from 2016-2017 school year
