These forms do NOT need to be filled out as part of the initial registration process. They will, however, need to be filled out and signed by a parent or legal guardian and delivered to your team's coach before your player will be allowed to participate in any team activity.
In addition to the signed concussion form (see below), parents/legal guardians of all TMYBA players must provide coaches with the following completed and signed forms:

In addition, parents/legal guardians of Heat players must provide their Heat coach with a concussion form (see below), a Medical Release (see above) and the following, all completed and signed:

Due to a Wisconsin law passed in 2012, concussion forms are required for all youth sports activities. Parents/legal guardians must read the Parent Concussion and Head Injury Information document (below) and have their players read the Athlete Concussion and Head Injury Information document (below). We realize some of our youngest players may not be able to read or fully comprehend the information. We will leave that decision to the parents. Parents/legal guardians and players must sign the third page of the Parent & Athlete Concussion Agreement and return the signed form to your team's coach before the player will be allowed to participate. Heat families will need to fill out and sign a concussion form for both their rec coach and their Heat coach.

Parent & Athlete Concussion Agreement
All coaches are also required to read the Coaches Concussion and Head Injury Information document and sign a Coaches Concussion Agreement (below). Coaches should carry this form with them to all practices and games, along with the signed forms, releases and agreements for all of their players.
