Please follow the procedures listed here to acquire the 3 necessary clearances and concussion in sports certification.
Federal Criminal History Record Information (CHRI) Clearance
Federal (FBI) Criminal History: $36 The fingerprint-based background check is a multiple-step process, as follows:
1. Go to www.pa.cogentid.com and select DEPARTMENT OF EDUCATION. Select REGISTER ONLINE. This will take you through the process to register, pay and get a receipt to take to the Fingerprint site. Take to the fingerprint site: License and proof of transaction(receipt)

Monday – Friday – 9 am to 5 pm
Saturday – 10 am to 12:30 pm
After your fingerprints have been submitted, do not contact the fingerprint site. You can check your “proof of print” on the Cogent website, by going to: https://www.pa.cogentid.com/index_pde.htm. Click on Proof of Transaction (Receipts). The Administrator at Canon-McMillan will receive notification and be able to review the report.

Select "Submit a new record check" (you are considered a non-registered user) Following the steps. When it asks for Reason For Request - SELECT "School District".

Department of Public Welfare Child Abuse History Clearance (Act 151)

3. Select "Click here to download" This is the form to complete.
You may not submit this request on line. You must download the application, print it,
complete the form and submit the form with your payment, by U.S. mail. Enclose a $10.00 money order for each application. No cash or personal checks accepted. Agency or business checks are acceptable.
Clearance results will be mailed within 14 days from the date that the clearance is received. There will be no replacements after 90 days.
Indicate SCHOOL as the "Purpose of Clearance"

Senate Bill 200 Requires completion of state mandatory course on concussions
This is Free.
Go to website: nfhslearn.com. Select "Get Free Courses" and then select "Concussion in Sport". You will be asked to "Log in to order" You will be required to register (it's free). Complete on-line course (takes approx 20 minutes). After completion, print out the certificate and give to Coach Moeller. This will be turned in to the AD and kept of file.
Senate Bill 200 requires all coaches to do this EACH year. You may not coach/volunteer without it.
