Following are the steps needed to complete the registration process. Click on the links to print forms.
1. Complete the online 2019 Spring Registration.
2. Complete the District Athletic Participation Form and email to registrar@skyhawksfootball.com or bring it to the Athletic Office at Southridge.
3. All incoming freshman, incoming juniors and new players will need a completed 2 page medical release form signed by their doctor. Email to registrar@skyhawksfootball.com or bring it to the Athletic Office at Southridge.
4. Read and sign the 2019 Camp Waiver. Email form to registrar@skyhawksfootball.com or bring to the Athletic Office at Southridge.

NOTE: All team fees will be due August 6th. We will begin accepting fees online after July 1, 2019 at:

**If paying by cash or check, please bring to the Business Office at Southridge after July 1, 2019.
Make one check payable to "Southridge High School" for the following fees:
District Participation fee - $225
Camp fee -$95
Laundry fee -$25
Total amount Due - $345
