Physical Form-2025 Physical Form-Substitute forms will NOT be accepted. We will not accept any physical that is performed by a DC (Chiropractor) or RN (Registered Nurse). Forms that DO NOT have the Doctors Stamp will not be accepted.
No player is allowed to participate in SCJAAF without having a current physical on file with their chapter dated no earlier than 4 months (March 21, 2025) prior to the season start date. Physicals done by a Chiropractor or Registered Nurse will NOT be accepted
