Responsible Alcohol Service Training

     1.    Go to: http://levy.restaurant.org/

       2.      Enter the password:  levy

       3.    Click “yes” when asked if Levy Restaurants is correct

       4.    Enter personal data and employment history

                         a.        Job Title:  select “Non-Profit”

                          ·        Volunteers/NFP Groups will select, “Non-Profit”

                          b.        Non-Profit Group Name:  "PAPILLION RECREATION ORG."

                   c.         Location Code, select:

                                    ·       “NE CenturyLink Center Omaha” or

                                     ·       “NE TD Ameritrade Park”

       5.    Click “Begin” in the lower right corner

       6.    Read all materials and click “next” to continue to the next page

       7.    After reading all materials, you will then be able to take the test

        8.    Once you complete the test and submit your answers, it will ask if you ould like for your supervisor to receive a copy.  In order to receive credit for taking this online class, please enter:

                 ·       If you do not enter this email address, we will not receive your results!

                           We will print your test results as proof of completion of the

                  on-line training.  You will then be scheduled to attend the next Live    Responsible Alcohol Service Training Class.
