WHAMO - Women's Hockey Academy of Mississauga ON

As a participant in the Women's Hockey Academy of Mississauga ON elsewhere referred to in this statement as WHAMO, I acknowledge that ice skating and hockey involve inherent and significant risk of injury.  I am aware of the risks and I freely accept and fully assume all responsibility for all risk and possibilities of personal injury.  I agree to release WHAMO, its' officers, directors and volunteers from all claims for injury or damages suffered as a result of my participation in the WHAMO.  I further agree to indemnify and save WHAMO, its' officers, directors and volunteers harmless from all claims by any party in any way resulting from my participation in a WHAMO activity.  By registering as an individual or as a member of a team, I acknowledge and provide my approval  for my name,, contact information (phone number and e-mail address) as well as individual statistics to appear on the WHAMO Website.  I acknowledge and accept the foregoing statement.

An indemnification statement such as the above is included in all our register forms and must be acknowledged in order to submit the form for registration.
