Please provide the following items to complete registration. All items are due no later than your team’s gear issuance day or gear will not be issued.

• 2026 Mt. Baldy Sports Physical & Medical Form (Pop Warner Form)

• 2026 National Pop Warner Physical Fitness and Medical History Form (Pop Warner Form)

• 2026 Participant Contract/Parental Consent (Pop Warner Form)

• 2026 Scholastic Eligibility (PW Form)

• 2025-2026 Report Card

• Current proof of residence ( Utility Bill)

• Birth certificate (ONLY FOR NEW PLAYERS-original kept until certification is complete)

• 2026 Parent Manual signed (Issued electronicaly in July)

• League Donation Registration Requirement: Each Athlete: 1 case of Gatorade (24-20 oz. bottle case) & 2 cases of soda (18 count-12 oz. cans-cases) Coke/ Pepsi products, OR $50.
