Team Check-in Requirements:

The items below NEED TO BE scanned and emailed to me prior to the tournament. Let me know if you will be bringing any of the documentation in person at check-in. The head coach MUST still check in with me at least 45 minutes prior to your first game by the gray barn at Riverside Park where I will present you with a set of free programs, shirt orders, and any other important information.
    1.    Official USA/ASA Roster (signed)
    2.    2021 USA/ASA Sanction Card with ASA number
    3.    Proof of Insurance
    4.    At least one person has ACE/Safe Sport certification-must show card/certificate
    5.    All adults in the dugout have a cleared background check-must show cards
    6.    All adults in the dugout have completed Ohio concussion certificate-must show certificate
