Agreement to Code of Conduct, Liability and Release Waiver

Please read this form carefully and be aware in registering yourself or your minor child/ward for participation in Avon Township Youth Baseball (“ATYB”), you will be waiving and releasing all claims for injuries and/or sickness you or your minor child/ward might sustain arising out of ATYB programs and you are agreeing to conform to the ATYB code of conduct.

• My child, spouse (or any relation of) and I agree to conform (where applicable) to all items listed in the ATYB Youth Code of Conduct and Adult Code of Conduct and I have read and understand the ATYB policy and agree to follow the guidelines set forth or face any penalty subject to any offense that I or my family member has committed.   In addition, I will follow, and direct my child/ward to follow the ATYB posted rules in connection with the safety guidelines for COVID-19.
•  I agree to waive and relinquish all claims my minor child/ward or I may have as a result of participating in the ATYB programs against Avon Township Youth Baseball, the Officers, Board of Directors, the Coaches, the Umpires, other Players and their Successors.
•  I understand that ATYB cannot prevent the possible transmission or contraction of COVID-19.
•  I do hereby release and discharge the Avon Township Youth Baseball, the officers, board of directors, coaches, umpires, other players, their successors, Avon Township, Round Lake Area Park District, Grayslake Park District, agents, servants and employees from any and all claims from sickness, including, without limitation, COVID-19,  injuries, damage or loss which I or my minor child/ward may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with the activities of ATYB.
•  I further agree to indemnify and hold harmless and defend Avon Township Youth Baseball and its officers, agents, servants and employees from any and all claims resulting from sickness, including, without limitation, COVID-19,  injuries, damages and losses sustained by me or my minor child/ward arising out of, connected with or in any way associated with the activities of ATYB.
•  Photographs are periodically taken of participants during the season.  These photos may appear on Avon social media sites and/or other outlets.  By signing up to play baseball and/or softball, you authorize ATYB to photograph your child’s participation without prior notice, permission, and without any compensation due to you.
•  In the event of any emergency, I authorize ATYB officials to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for me or my minor child/ward’s immediate care and agree that I will be responsible for payment of any and all medical services rendered.

 I have read and fully understand the above Program Details, Codes of Conduct, Waiver and Release of ALL Claims and give permission to secure treatment.
