Apparel Order Form and Instructions
Mainstreet will print Blaze apparel 1 time per month
around the 1st of each month.

Please send an email to Maureen Workman at
mssports@sbcglobal.net

to place your order. Include your contact number
and Maureen will contact you to take credit/debit
card payment over the phone.

We will have a Blaze representative pick up apparel
when it is ready and it will be distributed to the Blaze
Board representative for each team.

If you want something other than what is listed please call
Maureen at 812-384-8290. Her prices are good and they
are easy to work with.
