    In order to participate in a sport in any manner, the completed OHSAA Physical form and Final Forms registration are due in the athletic office prior to the first conditioning or practice. *Fee payment must be made prior to the first scrimmage or contest.

    In order to participate in a sport in any manner, the completed Final Forms registration are due in the athletic office prior to the first conditioning or practice. Log in and sign the forms for this year. Make sure the sport is selected as well.

  • Pre-participation Physical Evaluation Form 2018-2019
    All athletes are required to have a current physical on file in the athletic office prior to tryouts and/or the first day of mandatory practice. Physicals can be turned in to the athletic secretary in the main office, faxed to 740-657-4199 or emailed to Lisa_Daragona@olsd.us.
