Incident Reporting

In an effort to improve the process of reporting any incident that occurs during a scheduled TCYBS game, you must submit the details of the incident using the new TCYBS Incident Report form. The form should be completed the same day the incident occurs and submitted to the TCYBS president within 48 hours. During the course of investigating an incident, additional people may be requested to submit an Incident Report. If you are contacted for a follow-up, you will be provided a deadline for submission of the report.

You are requested to submit the Incident Report using the on-line form. When submitted, the incident details are emailed to the TCYBS president. You will receive a confirmation email that your Incident Report has been read by the TCYBS president.

Alternatively, you can submit a paper copy of the report available from your local president or by printing the Incident Report PDF. The PDF is a fill-in form, so you can enter the information into the form before it is printed. NOTE: the information entered into the PDF cannot be saved on your computer, so be sure to print the form before closing it.
