2018 Add a player to a registered Volleyball team

Complete one form for each new player you are adding to your roster. Remember there are only 15 players allowed on each team.

This form should only be completed if your team is officially registered. That is, you have submitted a roster form and the recreation department has confirmed that your registration is complete.
Please check over your work above. Make sure that all fields with a * are complete.

Please press the submit button below. If you have completed all of the necessary information, you will be taken to a summary page. You can print this form at the summary page. The team captain will receive a copy of this registration.

This player cannot play until Community Pass registration and payment have been confirmed by the rec department. The recreation department will send an e-mail when this has been confirmed.

As rosters are often slow to be updated, please bring the Recreation Department confirmation e-mail to the games until the player's name appears on the official roster..
