Field Reservation Form

This form is used to reserve a Lower Dauphin field for practices and/or games. The initial schedule for each team will be entered by the field scheduler after attending the Teener scheduling meeting and/or receiving the initial schedule from the Legion coach and the Twilight coach.

Please complete this form and submit. Your request will be processed within 24 hours and you will receive a confirmation email with information and any instructions you need to be aware of.
