ELIGIBILITY CRITERIA:

All three criteria must be met to be eligible for consideration. Proof of GPA and student enrollment is not required at the time of submission, but the Club reserves the right to request proof from applicant upon selection and prior to receiving the award.

Participated 4 years or more in Pasadena Baseball Club. *

Grade Point Average of 2.5 or better (on a 4.0 scale). *

Enrolled as a full-time student in coming fall semester at the university/college/school named above. *

SELECTION CRITERIA:

* List all high-school sports, clubs, groups, etc. that you participated in athletic sports and activities you participated in during your high school career. You may include non-school related athletics or activities::

5. SHORT ESSAY:

* Share some memories you have of playing baseball or other participation with the Pasadena Baseball Club. Use about 100 words - but no more than 200 words::
