* Incident Description:

Based on your umpiring experience, please indicate your estimation of the severity of this incident. For example, an adult ejected from a game but who left the game immediately without further incident should be considered at least a 2 (moderate to minor), but possibly a 3 (moderate) or higher if they did not leave the field immediately.

Please note that the PBC Sportsmanship Board or the BOD will determine any action to take as a result of this incident.
