VOLUNTEER APPLICATION (COACHES)

Thank you for your interest in volunteering positions. All applications will be considered and may be contacted for an interview. You are volunteering for a position in which you are in direct contact with participants. Under Megan’s Law, a full background check will be conducted. Background checks are $19.95 and paid for by the applicant once considered for the position. If you have any questions, please email Vincent Delgado at info@fontanatrojans.org.

By filling in this information, I give my permission for this organization to conduct a background check on me, which may include sex offender, child abuse, and criminal history backgrounds.

Participants volunteering must realize they hold a position of trust and responsibility in a youth program, which deals with a sensitive and impressionable period in a child’s development. All participants must have patience, understanding, and good communication skills with both children and adults.

All coaches and athletic directors will be required to complete a
mandatory concussion certification. Please go to... nfhslearn.com/courses ...and select the free course "Concussion in
Sports". Your completed certificate must be returned to
info@fontanatrojans.org within 72 hours of submitting your volunteer
application.

I understand if I am selected as head coach, it’s mandatory I assist with
the recruitment of participants and all Fontana Trojans Events.

I agree to comply with all by-laws, rules and regulations of the Southern California Junior All American Football Conference and Fontana Trojans Football and Cheer Program. I understand I can be removed from this organization if the above is violated.
