ACKNOWLEDGEMENT OF TRAINING

I acknowledge that I have read and understand the CFYFL SafeKids Child Abuse Molestation Risk Management Program and voluntarily agree that as a condition of future participation, employment, or involvement in this organization, I will abide by all the terms, conditions, policies, and procedures contained within this program.

If I violate the policies, regulations, or spirit of this program, I will indemnify and hold harmless the sports organization, its employees, board members, volunteers, and officials from any and all liability including negligence and any intentional tort claims.

VOLUNTEER COMMITMENT AGREEMENT

I understand that my role as a volunteer for the CFYFL and affiliated associations/teams is a significant responsibility and will make it a priority.

I look forward to working with this association and, like the others I will:
(1) Support the Mission, Vision, Values and Goals of the organization.
(2) Offer my expertise to help ensure the health and success of the organization.
(3) Contribute to fundraising and program activities, including registration events, introductions to potential donors, sponsors and grant makers each full year I serve.
(4) Work with the rest of the team to communicate the organizations role to our most important audiences.
(5) Attend in person at least 90% of the meetings held each year that I serve and whether I attend or not, will continually communicate with the team and the main coordinator to ensure I understand all current affairs. (Coaches Only)
(6) I understand as a Head Coach or coach I /we are responsible for all actions, requirements and expectations of the team at all times.

I understand that I serve in this coaching and/or volunteer position at the convenience of the CFYFL Board of Directors and the Board can terminate me from the position if they do not believe that I am upholding or abiding by the rules of the association or conduct myself in a manner befitting a youth volunteer and role model.

I understand & agree not to use or take any CFYFL participants email addresses, information or paperwork at anytime or use them with another organization. I understand if I violate this agreement the CFYFL can seek legal action against me. *

I have read and understand the above statements and agree to commit myself to the program for the CFYFL Football & Cheer Program and it affiliated associations. *

CONSENT/RELEASE

I authorize and give consent for the sports organization referenced above to obtain my personal information.

This includes, but is not limited to employment records/employers references; criminal background records/information; criminal background checks/fingerprints; driving record check, financial bankruptcy information, coaching experience, personal references, and addresses.

I authorize this information to be obtained either in writing, via internet, or via telephone in connection with my volunteer application.

I understand that my position is contingent upon adverse information about my background or character not being uncovered upon the performance of the above referenced checks. I also understand that regardless of my prior volunteer activities on behalf of the sports organization, that the sports organization is not required to allow my continued participation.

I agree to hold harmless and indemnify from liability the sports organization and its directors, officers, employees, and volunteers from all liability arising out of the use of the information that is uncovered in the above referenced checks.
