2019 Girls EOY Tournament Registration Form

CONCUSSION TRAINING:
All coaches are required to complete the required concussion training. Please tell you coaches to print the certificate at the conclusion of the training and bring it with them to the tournament.

TERMS OF REGISTRATION:
Once you've registered and the schedule is made, a failure to show or cancel is extremely disruptive to all involved including the officials whom expect payment.
If you have registered for the tournament and fail to show or cancel any or all of your games, your program will be expected to pay the official fees, for both your and the opposing teams, for each game cancelled.
