NEIL LEWIS GOLDEN YEARS SENIOR SOFTBALL LEAGUE APPLICATION

This application is for players age 62 and over with some exceptions. New players must submit a copy of their driver's license or other proof of age. The application fee is $60 if you already own a pair of league shirts and a hat or $105 if you do not already have league shirts and hats. There is a $30 Application Fee for pickup and reserve status each season. Players not drafted will have the option of withdrawing from the league and getting a full refund or agreeing to become a reserve/pickup player for a $30 Application Fee. Players can fill out applications for pickup games and reserve player status at any time. Actual season draft dates will be announced. Players registering after that date will be placed on a reserve list once the league receives the $30 Application fee. NO REFUNDS will be paid to drafted players. The two seasons will have a luncheon. Players will be required to fill out an application for each season they want to join as a drafted player or a reserve/pickup player.

NEIL LEWIS GOLDEN YEARS SENIOR SOFTBALL LEAGUE RELEASE AND WAIVER OF LIABILITY
By submitting this application, I hereby acknowledge that I am a member of the Golden Years Senior Softball League composed of men and women mostly 62 years or older with a few exceptions where younger players are allowed to join and participate. And that each player is aware of and acknowledges that the Golden Years Senior Softball League does not carry a policy of insurance to insure against injuries to a players physical person or property and each player understands that he or she agrees to an assumption of risk while playing softball and further agrees to save and hold harmless for themselves and any spouse, any heirs, any assigns, Golden Years Senior Softball League, its sponsors, managers, coaches, commissioners, administrative personnel, league teams or fellow players and the Palm Beach County Parks and Recreation Department and Palm Beach County from any and all liability for a players health, safety or injury resulting from their participation in the Golden Years Senior Softball League programs, activities or softball games. Each player further warrants that they have no physical condition or physical restriction which would prohibit their participation as a member of the Golden Years Senior Softball League. Each player further agrees to take full and complete responsibility for this action. In addition, each player further agrees to conduct themselves in a proper manor beneficial to the league and in the event any player conducts themselves in any way detrimental to the league, they may be disciplined, suspended or expelled for a period of time duly within the discretion of the Commissioners. In such event, each player agrees to save hold harmless for themselves, their spouse, heirs and assigns, Golden Years Senior Softball League, its sponsors, managers, coaches, Commissioners, administrative personnel, league teams or fellow players and the Palm Beach County Parks and Recreation Department and Palm Beach County from any and all liability as a consequence of said discipline, suspension or expulsion.
